INDEPENDENT SERVICES AGREEMENT

THIS AGREEMENT is made and entered into at (City, State), this ______ day of _________, YYYY between CLARA BARTON, R.N. and Legal Nurse Consultant, doing business as MEDICAL LITIGATION SUPPORT SERVICES, ("BARTON"), and the law firm of Smith, Bruce and Tuttle ("CONTRACTOR"), who agrees as follows:

1. CONTRACTOR engages BARTON as an independent contractor to review and analyze the medical records of CONTRACTOR'S client (referenced above) and provide a written narrative summary of those records.

 

Additionally, BARTON will recommend one or more potential experts in the specialty(s) of _______________ who may be available to review this case and provide expert forensic testimony.

 

 

2. BARTON shall determine the method, details and means of performing the services. BARTON is under no obligation to perform services for a specified number of hours. CONTRACTOR shall deliver to BARTON copies of the client's medical records.

 

 

3. BARTON will deliver the required written narrative to CONTRACTOR within _____ days/weeks of receipt of the client's complete medical records. BARTON will make best efforts to deliver the narrative on this patient on or before _____, YYYY.
